If you were born in Lucas County and need to order a copy of your birth certificate, click on
birth certificate to download the form off the web. A death
certificate can also be obtained by clicking on death certificate and downloading the form. Fill in the appropriate information and mail it back to us. If you wish to pay with a credit card, please go to this site http://vitalchek.com and click on Ohio then go to Lucas County.
The fee for a certified copy of a certificate is $20.00 per copy. Due to changes in State law, we can no longer provide uncertified copies of birth or death certificates. Send us a self-addressed stamped envelope. Our address is Vital Records, Toledo-Lucas County Department of Health, 635 N. Erie St., Toledo, Ohio 43604.
If you come to our office, in most cases, you should leave with your certificate. If you send for your certificate, upon receipt of your request, we will mail it back within 2 business days. You must allow mail time both ways
